Payment & Insurance Information
for PT Services
At Performance Sports Physio Clinic, we operate as a hybrid clinic, offering both in-network insurance services and cash-based care options. This model allows us to provide flexible access to care while still maintaining our standard of high-quality, one-on-one, personalized treatment.
In-Network Insurance Plans
We are currently in-network with a growing number of insurance providers, including:
- Medicare
- Medicaid (Health First Colorado)
- Anthem Blue Cross Blue Shield (multiple plans and networks)
- University of Colorado Health Plans
- UnitedHealthcare Medicare Plans
- TriWest / VA Community Care
- Aetna
- GEHA
- Meritain Health
- Golden Rule
- And many other commercial and third-party administrators (TPAs)
Our in-network list is continuously expanding as we partner with additional insurance providers.
Because coverage varies depending on your specific plan, we recommend contacting us so we can help verify your benefits and eligibility.
Cash-Based Services (Out-of-Pocket Care)
We also offer cash-based services for patients who:
- Prefer not to use insurance
- Have high deductibles or limited coverage
- Want performance-focused or specialized care beyond typical insurance limitations
This option allows for:
- Longer, uninterrupted one-on-one sessions
- Fully personalized treatment plans
- Greater flexibility in care approach and frequency
Out-of-Network Reimbursement (Superbill)
If you choose to pay out-of-pocket, we can provide a Superbill that you may submit to your insurance provider for potential reimbursement.
Depending on your plan, services may:
- Apply toward your out-of-network deductible
- Qualify for partial reimbursement
- In some cases, be reimbursed in full
Reimbursement is determined by your individual insurance policy.
