  2. Filling out the Online Intake Form (for Physical Therapy Service)
        • Once you make an appointment, you will receive a welcome email with a link to the Online Intake Form. We would encourage you to fill it out at least a day before the appointment day so that we don’t waste your one-on-one appointment time by asking you the questions.
  3. The First Session at the Clinic
        • Please wear an appropriate clothing and shoe wears depending on your appointment type, either it is for body conditioning/workout or for physical therapy/rehabilitation.
  4. Now, You Successfully Establish Care with us, Performance Sports Physio!
        • We are a multi-disciplinary care team including Physical Therapist, Athletic Trainer, Strength and Conditioning Specialist, Acupuncturist, Rolfer®︎, Chiropractor, and Massage Therapist. 
        • As your care team, we will help you reach the goals and solve the problems you have.

Insurance and Medical Billing

At Performance Sports Physio Clinic, we operate primarily as a cash-based clinic, allowing us to provide longer, higher-quality, one-on-one care without the limitations or restrictions of typical insurance plans.
This model ensures you receive truly personalized, effective treatment focused entirely on your needs—not on insurance guidelines.

Insurance We May Work With

While we are primarily cash-based, we may be able to work with certain insurance plans, including:

  • Medicare / Medicaid

  • TriWest / TRICARE

  • Anthem Blue Cross Blue Shield

  • Cigna

Coverage varies based on your individual plan. If you have questions about eligibility or benefits, we are happy to assist you in checking your coverage.

Out-of-Network Reimbursement (Superbill)

If your plan is out-of-network, we can provide a detailed invoice known as a Superbill.
You may submit this directly to your insurance provider for potential reimbursement. Depending on your policy, these visits may:

  • Apply toward your out-of-network deductible

  • Qualify for partial reimbursement

  • In some cases, be reimbursed in full

Please note that reimbursement amounts depend entirely on your insurance plan.
